About the Job
The RN Embedded Care Coordinator, Behavioral Health serves as a vital member of Foundation Counseling & Wellness, coordinating care and supporting patients seeking behavioral health services. This role partners closely with providers, patients, families, and community organizations to ensure timely access to care, effective care transitions, and a seamless patient experience.
Using strong clinical judgment and a patient-centered approach, the RN Embedded Care Coordinator helps patients navigate behavioral health resources, supports care planning, assesses urgent needs, and connects patients with the appropriate level of care both within Southern New Hampshire Health and throughout the community.
What You'll Do
- Coordinate care for patients referred to behavioral health services, ensuring timely access to appropriate providers and resources
- Conduct patient assessments and apply the nursing process in person, via telephone triage, and through telehealth platforms
- Assess crisis situations and facilitate appropriate interventions, referrals, and follow-up care
- Serve as a liaison between patients, providers, community agencies, and health system partners to support continuity of care
- Maintain knowledge of behavioral health and social service resources throughout Greater Nashua and Southern New Hampshire, connecting patients with long-term community support when needed
- Support care coordination activities, including patient screenings, prescription refill workflows, prior authorizations, and controlled substance agreement tracking
- Assist patients in understanding insurance benefits, coverage limitations, copayments, deductibles, and financial policies
- Promote patient safety, quality outcomes, access to care, and patient satisfaction through collaborative care management practices
- Utilize Epic and other clinical systems to document care, track patient needs, and support evolving behavioral health initiatives
Who You Are / Requirements
Minimum Qualifications
- Associate Degree in Nursing (ASN) from an accredited nursing program
- Current Registered Nurse (RN) license in the State of New Hampshire
- Current BLS/CPR certification
- Two (2) years of professional nursing experience in a relevant clinical specialty - Behavioral Health
Preferred Qualifications
- Bachelor's degree in Nursing, Psychology, Sociology, Human Services, or a related field
- Experience in ambulatory care, behavioral health, care coordination, case management, or population health
- Certification in Ambulatory Care Nursing (RN-BC), Care Coordination (CCTM), or a related specialty certification

- Ability to work independently while collaborating effectively with interdisciplinary teams and community partners
- Proficiency with EMR systems, including Epic, and Microsoft Office applications
