This department includes a dedicated team of Case Managers, Social Workers, and Interpreters that ensure that all care is equal, and our patient's dignity is maintained above all else.

About the Job:

The Case Management Assistant will work collaboratively with the case management team (Case Managers, Social Workers and Utilization Review) to ensure seamless patient care and efficient administrative operations. This role will demonstrate organizational ability in coordinating administrative workflow for the case management team and assisting with care plan implementation under the supervision of the care management staff.

What You’ll Do:

  • Ensure compliance with regulations, will verify insurance.
  • Arrange transportation and facilitate post-acute care referrals.
  • Provide administrative support to the case management team. Receive and screen incoming requests to the team, acting when appropriate and providing consistent follow-up as necessary.
  • Compose, type and proofread correspondence making necessary corrections when requested.

Who You Are:

  • High School Diploma required.
  • A minimum of two (2) years of administrative support, preferably in a customer service facing or healthcare setting, required.
  • Basic understanding of medical terminology & HIPPA preferred.
