Part-Time Palliative APRN Opportunity
Foundation Community Care – Part of Foundation Medical Partners / Southern New Hampshire Health

Foundation Community Care, a member of Southern New Hampshire Health, is currently seeking a Part-Time Advanced Practice Registered Nurse (APRN) to join our Palliative Care team.

Position Details:

  • This will be 50% Inpatient and 50% Outpatient
  • Schedule: 20–24 hours per week
    • Flexible options: Three 8-hour days or two 8-hour days and one 4-hour day
  • Location: On-site only; remote work is not available for this role

Qualifications:

  • Minimum of 1–2 years of clinical experience in Primary CareInternal Medicine, or Hospice is required
