About the Job

As an Ambulatory Registered Nurse in our Diabetes & Endocrinology practice, you will play a vital role in delivering specialized care to patients managing chronic endocrine conditions.

You will collaborate closely with endocrinologists, advanced practice providers, and care teams to provide evidence-based, patient-centered care, including in-office care, telehealth services, and chronic disease management support.

What You’ll Do

  • Deliver comprehensive nursing care using the nursing process (assessment, planning, intervention, evaluation) with a focus on chronic disease management
  • Support patients with diabetes care, including blood glucose monitoring, medication adherence, and lifestyle management
  • Educate patients on:
    • Insulin administration and injectable therapies
    • Continuous glucose monitors (CGMs) and insulin pumps
    • Nutrition, weight management, and lifestyle modifications
  • Collaborate with providers to develop and implement individualized endocrine care plans
  • Administer medications, vaccines, and perform point-of-care testing (e.g., A1C, glucose checks)
  • Provide telephone triage and telehealth support, addressing patient questions and symptom management
  • Document care accurately in the EMR and assist with pre-visit planning
  • Monitor and follow up on lab results, medication adjustments, and care plan adherence
  • Serve as a resource and mentor to clinical staff; assist with onboarding and education
  • Respond to urgent clinical concerns in alignment with established protocols

Who You Are:

  • Associate of Science in Nursing (ASN) required; Bachelor of Science in Nursing (BSN) preferred.
  • Current Registered Nurse license in NH (or NLC state/MA) and BLS certification required.
  • 1+ year of professional nursing experience preferred; ambulatory care experience a plus.
  • Strong knowledge of nursing principles, EMR systems, and patient-centered care models.
  • Skilled communicator with excellent time management, adaptability, and teamwork abilities.
  • Compassionate, detail-oriented, and able to thrive in a fast-paced, collaborative environment.
