About the Job:

The Registered Nurse Ambulatory is responsible for providing professional nursing care to patients in an outpatient practice setting. Working collaboratively with providers and interdisciplinary team members, the RN delivers patient-centered care through in-person visits, telephone triage, and telehealth services. This role supports patient education, care coordination, chronic disease management, preventive care initiatives, and clinical procedures while ensuring high standards of quality, safety, and compassion.

What You'll Do:

  • Provide comprehensive nursing care utilizing the nursing process, including assessment, planning, intervention, and evaluation.
  • Conduct telephone triage and support patient care through telehealth and in-person interactions.
  • Collaborate with physicians, advanced practice providers, physician assistants, and other healthcare professionals to develop and implement patient care plans.
  • Educate patients and families on medications, disease management, wellness, nutrition, and preventive care.
  • Assist providers with examinations, treatments, procedures, medication administration, vaccinations, and specimen collection.
  • Perform pre-visit planning, identify care gaps, and support chronic disease management initiatives.
  • Accurately document patient care and clinical activities within the electronic medical record.
  • Serve as a clinical resource and mentor for team members while fostering a positive and collaborative practice environment.
  • Maintain compliance with safety, infection prevention, privacy, and regulatory standards.

Who You Are / Requirements:

  • Associate Degree in Nursing (ADN) from an accredited nursing program required; BSN preferred.
  • Current Registered Nurse license in the applicable state of practice (NH, NLC state, MA, or both).
  • Current BLS/CPR certification required.
  • One year of professional nursing experience preferred; ambulatory or outpatient care experience preferred.
  • Strong clinical assessment, communication, organizational, and time-management skills.
  • Ability to prioritize, multitask, and thrive in a fast-paced healthcare environment.
  • Proficiency with EMR systems and standard computer applications.
  • Passion for delivering exceptional patient-centered care and improving patient outcomes.
