About the Job:
The role of the nurse navigator along the continuum of care is bidimensional in nature with a patient centered (advocate, empowerment with education, and psychosocial support) and health system (member of the multidisciplinary team) orientation to deliver timely, seamless care. Across the care continuum, the role of the nurse navigator is to identify and address barriers to care (utilizing the 5 steps of the nursing process); empower patients with appropriate education and awareness of health literacy so they can make informed decisions; offer psychosocial support and access to resources; advocate for the unique needs with a cultural awareness of each patient regarding the use of facility and community resources; encourage patients to be engaged in their care planning; streamline care path transitions and logistic issues (diagnosis, treatment, survivorship, and end of life); and liaise between clinical and nonclinical specialists in the multidisciplinary care team.
What You’ll Do:
- Serve as the key point of contact for patients, families, and providers throughout the oncology care continuum.
- Assess patient needs and connect them with appropriate medical, emotional, financial, and community resources.
- Provide education on diagnosis, treatment, side effect management, and survivorship planning.
- Advocate for patient-centered care that reflects cultural awareness, health literacy, and individual goals.
- Collaborate with physicians, nurses, and support staff to coordinate care and reduce delays or gaps.
- Track patient outcomes and contribute to quality improvement of the navigation program.
- Support transitions between active treatment, survivorship, chronic disease management, and end-of-life care.
Who You Are:
- Graduate of an NLN-approved nursing program; Master’s degree required within 3 years of hire.
- Licensed RN in New Hampshire or Compact State.
- ONN-CG or CBCN certification (if breast navigation specific) required.
- BLS certification required.
- 3–5 years of clinical nursing experience (oncology experience strongly preferred).
- Strong communication, advocacy, and critical thinking skills.
- Able to work independently and collaboratively with inter-professional teams.
