What You’ll Do:

  • Assess patient’s condition and obtain pertinent elements of past and current history.

  • Perform vital signs and other wound/ostomy screenings per provider guidelines.

  • Conduct advance assessments of acute and chronic wounds, ostomies, and fistulas.

  • Triage patient calls per practice guidelines.

  • Evaluate the continuing wound care needs of the patients and work in collaboration with the clinical care team and other community resources to facilitate/direct an appropriate plan of care.

  • Order appropriate tests/medications as instructed by the provider and assist with obtaining precertification/authorization as needed.

  • Successfully manage clinic flow rooming patients; assist in preparation for procedure and obtain appropriate authorizations/consents.

  • Organize and maintain inventory of wound/ostomy supplies and medications.

  • Inspect and maintain eye wash station supplies, office emergency kits, and refrigerator/freezer temperature log per guidelines.

  • Assist the provider with examinations, diagnostic procedures, and treatments when a provider sees wound patients.

  • Perform specific procedures/treatments such as wraps, VACs, and complex dressing changes and manage complex ostomies and fistulas.

  • Respond properly to emergency codes.

  • Promote wellness by providing education materials and communicating provider advice/instructions.

  • Mentor and train new clinical staff.

  • Educate patient and families on self-management activities such as wound care dressings, medications, nutrition, activities, health prevention, and management of co-morbidities.

  • Consult with Wound Care provider as needed. Round with Wound Care provider.

  • Competent in assessment of peripheral pulses using Doppler.

  • Perform other duties as assigned.

Who You Are:

  • Education:Bachelor’s degree required. Bachelor’s degree in Nursing preferred.

  • Licensure/Certification:Wound Care certification required. Ostomy certification required. BLS certification required.

  • Experience:A minimum of three-five (3-5) years of related nursing experience required. Wound care experience preferred.

  • Skills:Proficiency in Microsoft Office Suite. EPIC knowledge preferred.
