We are seeking a compassionate and experienced Family Medicine Nurse Practitioner (FNP) to join our dynamic team at Foundation Medical Partners / Southern NH Health. In this role, you will play a vital role in providing comprehensive care to a diverse patient population in Pepperell, MA.
- Collegial practice with solid APP support
- W2, employed position with Foundation Medical Partners
- Not-for-Profit, 501(c) Organization
- 32-36 Patient Hours / plus admin hours per week
- Clinical Affiliate of Massachusetts General Hospital
- Convenient Southern NH/MA Border Location, minutes to the mountains or ocean

Qualifications:
- Master’s degree in nursing
- 2+ years of Family Medicine experience
- Strong clinical skills, includingassessment, diagnosis, and treatment planning.
- Excellent communication, collaboration, and interpersonal skills.
- Proficiency with EPIC EMR preferred
- Ability to work independently and as part of a team.
