Our providers specialize in the prevention, diagnosis, and treatment of gastrointestinal conditions, including GERD, inflammatory bowel disease, liver disorders, colorectal conditions, and other digestive health concerns. We partner closely with our patients to provide comprehensive, patient-centered care that supports long-term health and wellness.
About the Job:
Responsible for managing care for complex gastroenterology patients to promote effective education, self-management support, care coordination, and timely healthcare delivery. Collaborates with patients, providers, and interdisciplinary teams to develop individualized care plans and improve patient outcomes.
What You’ll Do:
- Provide care coordination and case management for patients with complex healthcare needs.
- Collaborate with providers and interdisciplinary care teams to develop individualized care plans.
- Educate patients on disease management, medications, nutrition, and self-management strategies.
- Coordinate transitions of care, including hospital discharges, specialty referrals, and follow-up care.
- Utilize population health tools and electronic medical records to identify and manage patients requiring additional support.
- Promote patient safety, quality outcomes, continuity of care, and preventive health initiatives.
Who You Are:
- Associate Degree in Nursing (ASN) or equivalent from an accredited institution required. Bachelor of Science in Nursing (BSN) preferred.
- Current New Hampshire Registered Nurse license and BLS/CPR certification required.
- Minimum of two years of professional nursing experience required.
- Ambulatory care, case management, population health, or care coordination experience preferred.
- Strong clinical assessment, communication, and care coordination skills.
- Knowledge of electronic medical records, chronic disease management, and patient-centered care principles.
- Ability to manage multiple priorities, work independently, and collaborate effectively with patients, families, and care teams.
